چکیده

We evaluated the accuracy of an erythromycin-clindamycin double-disk test (D-zone test) and an erythromycin-telithromycin D-zone test for detection of inducible resistance in isolates of beta-hemolytic streptococci with erythromycin resistance. The results of these tests were compared to results of a broth microdilution (BMD) induction test using combinations of erythromycin and either clindamycin or telithromycin. Of 29 erythromycin-resistant, clindamycin-susceptible isolates, 16 were positive by the erythromycin-clindamycin D-zone test; all of these demonstrated inducible clindamycin resistance by BMD. Twelve isolates were D-zone test-negative, did not demonstrate inducible resistance by BMD, and were positive for a mef determinant. Of 39 erythromycin-resistant, telithromycin-susceptible isolates, 13 were erythromycin-telithromycin D-zone test-positive, 19 questionably positive (unclear blunting of the zone), and 7 were D-zone test-negative. The erythromycin-telithromycin D-zone test result did not correlate with inducible resistance by BMD or the presence of an erm or mef gene. These results demonstrate that the erythromycin-clindamycin D-zone and BMD induction tests accurately detect inducible clindamycin resistance, but the erythromycin-telithromycin D-zone test is not reliable for detecting inducible telithromycin resistance.

Antimicrobial Susceptibility Patterns and Macrolide Resistance Genes of β-Hemolytic Viridans Group Streptococci in a Tertiary Korean Hospital

The aim of this study was to investigate antimicrobial susceptibilities and macrolide resistance mechanisms of beta-hemolytic viridans group streptococci (VGS) in a tertiary Korean hospital. Minimum inhibitory concentrations (MICs) of seven antimicrobials were determined for 103 beta-hemolytic VGS isolated from various specimens. The macrolide resistance mechanisms of erythromycin-resistant iso...

Antimicrobial susceptibility patterns and macrolide resistance genes of beta-hemolytic streptococci in Korea.

In 540 beta-hemolytic streptococci, the rates of resistance to tetracycline, chloramphenicol, erythromycin, and clindamycin were 80.0, 22.8, 20.2, and 19.1%, respectively. Of the erythromycin-resistant isolates, 63.3% had the constitutive macrolide-lincosamide-streptogramin B (MLS(B)) resistance phenotype, 23.9% had the M phenotype, and 12.8% had the inducible MLS(B) resistance phenotype. The c...

Induction of telithromycin resistance by erythromycin in isolates of macrolide-resistant Staphylococcus spp.

Staphylococcal isolates were examined for possible macrolide-inducible resistance to telithromycin. All macrolide-resistant isolates demonstrated telithromycin D-shaped zones. This result did not discriminate between resistance due to an efflux mechanism (msrA) or a ribosomal target modification (ermA or ermC). Inducible telithromycin resistance in staphylococci does not appear to be analogous ...

Inducible Clindamycin Resistance Is More Common in Lancefield groups C and G Streptococci Compared with group A in Clinical Specimens: A Single-center Experience, 2013 to 2016

Background. Current susceptibility testing recommendations for β-hemolytic streptococci outline testing for clindamycin resistance, including inducible resistance by a positive D-zone phenotype. However, few studies describe the prevalence of clindamycin resistance among invasive GCS and GGS organisms. This study aims to describe the prevalence of clindamycin resistance among GCS/GGS compared w...
